Form fields not visible

Hello,

We are having an issue with version 6.3.4 of the viewer. I’m not exactly sure on the version number, in package.json it shows 6.3.4 but when looking in chrome console when the viewer loads a document the UI version is 6.3.3 and core is 6.3.4. When the document loads not all form fields are visible, for example on a five page PDF we have, the form input fields on pages 1-3 work just fine but any on page 4 or 5 are not visible.

This problem does not exist in the previous version of the viewer (6.2.1?) (ui=6.2.0, core=6.2.1), all form fields are visible and work as expected on the same five page document.

For now we had to downgrade back to 6.2.1, looking forward to the new release.
