Issue with importing radio buttons in pdf

Detailed description of issue
When pdf is loaded into viewer , radio buttons are missing from it. Also , it throws an error on console saying ‘Error: undefined is not a valid destination type.’ Is this an error from your end. Can u pls confirm ?.

This issue is probably related to Bug on file upload in pdf js viewer - #4 by Logan. It will be fixed in next release
